Fixed /teamwork-preview and some other slash commands disappearing for some users.
Fixed Enter not opening an active background task or subagent while the prompt was in Vim insert mode.
Fixed attaching Ogg audio and video files such as .ogg, .opus and .ogv, which the model rejected because they were sent as the generic application/ogg.
