Fixes conversation restoration across Visual Studio restarts, resolves a chat input focus bug after agent file edits, and improves multi-turn diff review handling.

- Fixed the Antigravity tool window resetting to the home screen after restarting Visual Studio by persisting and restoring the last active conversation for the current solution or workspace.
- Fixed a focus issue where clicking the chat input box after the agent opened or modified files in the editor failed to focus the chat input and instead highlighted the sidebar toggle button.
- Fixed multi-turn agent edit reviews to preserve the original pre-agent file baseline across consecutive turns before a review is resolved, keep target editor buffers read-only while a diff review is pending, and match active edit sessions using case-insensitive file paths on Windows.
